At-risk patients follow their rhythm for 7 days with FibriCheck; where AF is confirmed, anticoagulation is discussed and initiated immediately.
Objective
To improve the atrial fibrillation detection yield in treatment-relevant patients while making the pathway more efficient for both patients and physicians.
Approach
The AF stroke prevention hub defines a group of at-risk patients who follow their heart rhythm for 7 days with FibriCheck. Normal results receive reassurance and self-management advice from the hub nurse. Where FibriCheck detects AF, patients are asked to visit for a confirmatory 12-lead ECG or 7-day Holter, and anticoagulation therapy is discussed and initiated immediately.
Outcome
The end-to-end pathway improves the efficiency and outcome of AF care. The Trust was recognised by the AF Association as an AF pioneer for the programme and was shortlisted for an HSJ digital award celebrating innovation in digitising and transforming healthcare.
