Approach

Chelsea and Westminster Hospital works with Patients Know Best (PKB) on a Same Day Emergency Care pathway that rapidly assesses, diagnoses and, where clinically appropriate, discharges patients the same day. FibriCheck’s remote cardiac monitoring aligns to usual care pathways, and data flows into PKB, the UK’s leading personal health record provider with over 1.5 million users.

Outcome

By enabling patient-reported and clinical data to be viewed holistically, the pathway shortens length of stay, creates much-needed bed capacity and provides better outcomes for both patients and the healthcare system.
