Delivering healthcare at home - The TeleCheck-AF project
This project demonstrates the remote monitoring possibilities of FibriCheck. Patients had to measure their heart rhythm during the 7 days leading up to their scheduled teleconsultation. With the data from these measurements, the physician could decide to start or adapt treatment.
Creating a nurse-led post-AF ablation follow up pathway
FibriCheck transformed the clinical pathway of the Essex Cardiothoracic Centre into a digitally-enabled pathway led by arrhythmia nurses in which ECGs and Holter devices were replaced. This translated into direct financial savings, but also into time savings, allowing for increased efficiency of the hospital service. In the future, this project will expand and FibriCheck will be used to follow up on patients who underwent a cardioversion procedure as well.
